Self referrals

There are a number of services locally that you are able to make self referrals to.

Hospital and community paediatric referrals

Where your child requires further assessment and support from health care services, we will arrange this for you following assessment by one of the GPs.

This may include emergency care and where you child needs same day emergency treatment, we will see you in the emergency clinic and arrange hospital admission if they need urgent treatment.

Hospital referrals

If following your consultation, the doctor decides you require hospital assessment or treatment, he or she will refer you to the hospital.

The practice uses the choose and book system for referrals, where patients are given a reference number and password and ring a booking line to make their own appointment.

We aim to process all referrals within ten working days, although urgent referrals will be done immediately.

Waiting time for the out-patient clinic and hospital treatment vary.

If you feel your condition is getting worse please contact the doctor. If appropriate they will request your appointment or treatment be brought forward.

Appropriate referral and investigation

Where your long term condition requires it, we will arrange for hospital referral for further investigation and treatment.

Referrals to other support services

We can arrange referral you to services such as:

  • Counselling
  • Addiction services
  • Smoking cessation support
  • Derby First Contact for social care support
