District nurses
We are very fortunate to have an excellent team of district nurses attached to our practice.
The district nurses can be contacted either by leaving a message with the reception staff or by ringing the district nursing liaison office on 01332 564 900.
Any requests made to reception staff for a district nurse to visit will be passed on for the district nurse to assess further.
Care coordinator
Emma Sumner is our practice based care coordinator.
She is based at the surgery on St Mark’s Road from Monday to Thursday.
Her role is to coordinate support around vulnerable adults at risk of a hospital admission.
